Patient Registration Forms
Appointments
We see patients by appointment only. New patients should schedule the first visit as a complete examination. The practitioners cannot treat you for a specific problem without knowing your medical history. If another physician has referred you to our office, please ask that physician to send any pertinent records to us prior to your appointment.
To help us serve you better, please plan to arrive 15 minutes early for your appointment to ensure accurate completion of registration forms.
We all love children, but please avoid bringing small children to the office if possible so we may devote our full attention to you.
If you need to cancel or reschedule an appointment, please call us as soon as possible. Occasionally, we will need to reschedule your appointment due to an emergency. If you are traveling a significant distance to our office, we encourage you to call to confirm the appointment.
Telephone Calls
Calls should be made during office hours so your chart can be accessed by our staff. Briefly tell the receptionist why you are calling, the best phone numbers and times to return your call, and whether we have your permission to leave a message on your answering machine or with another individual. Our medical assistant will return your call, who is educated to answer many general questions and find out more information for the practitioner.
In the case of a life-threatening emergency, call 911 or go to the nearest hospital emergency department. Our staff does not provide hospital services but can refer you to another physician if you require hospitalization. For less severe emergencies, call our office. The after-hours message will prompt you to leave a message for the on-call physician who will be paged immediately. If you do not receive a response after 30 minutes, please call again.
Patients with Disabilities
We are wheelchair accessible and will make every reasonable accommodation for patients with disabilities. Please inform our receptionist in advance so we can be prepared to make the appropriate arrangements. Reserved handicapped parking spaces are available near the office entrance.
Confidentiality
Your medical records are confidential. You may make an appointment to review your medical records, for which a fee may be charged. You may add written documentation to correct your medical records if necessary. We will not release copies of your records to others without your written permission unless the law authorizes or compels us to do so. Your written permission should include permission to release information regarding sexually transmitted diseases.
Billing and Insurance
We bill many insurance companies directly. If you have an insurance that we do not bill or if you have not supplied us with your insurance information, you will receive a statement that may be submitted to your insurance for reimbursement. Our policy is payment at the time of services for patients who have insurance with companies with whom we are not contracted.
You are responsible for any balance on your account regardless of insurance coverage. For your convenience, we accept both Visa and MasterCard. Contact our office at (206) 522-3330 if you have any questions or concerns about your account. We will be happy to assist you. There will be a $20 fee for each check payment that is returned as Non-sufficient Funds.
Insurance carriers routinely require that annual gynecological exams, which include a complete physical exam, be billed under preventive medicine codes. Please check your insurance coverage to determine if these services are covered and if co-pay is required. A specific problem or concern may need to be addressed on a separate visit. (Please do not ask us to change a diagnosis code after your visit has been billed.)
Our fees only reflect the services provided in our office. Lab specimens are sent to outside labs for processing. These labs will send you a separate bill for any services they provide. Please contact them directly if you have any questions regarding their fees or billing.
